What You're Agreeing To

By completing the waiver form, the parent or legal guardian acknowledges and agrees to the following terms on behalf of their minor athlete:

Assumption of Risk: Participation in soccer training involves physical activity that carries inherent risk of injury. The parent/guardian voluntarily accepts these risks on behalf of the athlete.

Liability Release: The parent/guardian releases Grassroots Greatness LLC, its coaches, staff, and volunteers from liability for injuries or accidents that may occur during training sessions, unless caused by gross negligence.

Medical Authorization: In the event of an emergency, Grassroots Greatness LLC staff may seek medical treatment for the athlete if the parent/guardian cannot be reached promptly.

Code of Conduct: The athlete and parent/guardian agree to respect coaches, teammates, and facilities, and understand that disruptive behavior may result in removal from sessions.

Photo/Video Consent: Grassroots Greatness LLC may take photos or videos during sessions for promotional use. Parents may opt out of this on the waiver form.
